The care plan is a means of communicating and coordinating the actions of a constantly changing nursing staff.
Clinical and care records include a wide variety of documents generated on, or on behalf of, all the health professionals involved inpatient care. This includes handwritten clinical notes and computerised/electronic clinical records.
