The term 'transition' is used in the Care Act to refer to the process of change for young people, and those around them, as they progress from childhood to adulthood. Transition is an essential part of human life and experience and can be a time of celebration, change and also challenge for young people.

This course forms part of the suite of e-learning courses that have been developed to support the implementation of part one of the Care Act 2014. It is based upon the Skills for Care training materials.
Upon successful completion of both modules you will be automatically awarded a certificate containing the course name, completion date, CPD hours and learning objectives.
The course contains additional resource materials, useful links and refresher guides.

This course is intended for:
• Practitioners directly involved in the planning and support of young people, including those with special educational needs (SEN) and complex needs, who will transition to adult care and support, and their carers
• Specialist information and advice workers
• Managers of both the above types of roles
• Managers and commissioners in local authorities with a responsibility for the provision and market development of services for young people who will transition to adult care and support, and their carers
