Transition Care have an exciting opportunity available for a Team Leader Position within existing and new services within the company.

Salary:37.5 hours - £9.55 per hour overtime paid at £7.83 per hour

Must be willing to be part of the out of hours on-call Rota.

TEAM LEADER JOB DESCRIPTION

REPORTS TO: Service Manager

JOB PURPOSE: To work as Team Leader of the team in the provision of care and support services for people with Mental Health needs/Learning Disabilities and/or behaviors that may challenge; which aims to maximize the potential of individuals.

THE PERSON: A positive, enthusiastic role model for staff who has the ability to see the "biggerpicture". Understands the importance of maintaining the service users character and encourages staff to maintain positivity around the service users when they present with behaviors that may challenge others.

EXPERIENCE

Required:

Evidence of management experience and knowledge of working in a health care setting.

Desired:

Minimum of Two years working in a similar setting

Education, Specialist knowledge and qualifications

Required:

Experience of a supervisory role

Good general education

Working knowledge of health and safety issues

Working knowledge of safeguarding procedures

NVQ Level 3 or equivalent in social care or management or willing to work towards

Have or willing to undertake all Mandatory training qualifications e.g. fire safety, medication, first aid, food hygiene, Manual Handling, and Health and Safety awareness.

Willing to undertake any training relevant to the Team Leader role.

Desired

Knowledge of regulatory framework of service provision including Care Standards Act and The Health and Social Care Act

Required Skills -

Team Leadership and Coaching: -

Contribute to staff appraisals and supervision process.

Team Management: -

Facilitate the day to day organisation within the service

Ensure appropriate staffing levels are maintained to provide effective care provision.

Manage staff sickness levels, organising necessary cover on a day to day basis.

Familiarize the team with service users needs and ensure the team address all relevant areas of care.

Organise and facilitate regular staff meetings.

Provide necessary organisational information to staff team.

Help keep the team focused and on track

Operational Management:

Able to put in place Support Plans, Risk Assessment and Risk Assessment Management Plans. Review and update these when necessary.

Responsible for completing weekly and monthly finance checks, medication checks, health and safety checks and reporting any issues to the Service Manager

Attend regular meetings as directed by management.

Maintain overview of daily records, incident logs and staff rota.

Ensure a high standard of presentation -clean and presentable house environment and maintenance.

Keep the management team informed of steps towards Service User outcomes and any ongoing issues; looking at ideas to solve problems.

Facilitate positive working relationships with service user’s family and professional team

To act in the best interests of the organisation and those for who it provides care.

Maintain quality and standards of the organisation in all areas of provision.

Maintain all areas of Health and Safety relevant to the service users, working environment and staff teams.

Monitor and support training within the staff team.

Main Responsibilities

1) Undertake expected working hours as indicated by company rota including nights and weekend duties, sickness cover and on call roster as required.

2) Assist in the implementation of the philosophies and policies of the company to minimise the effects of the Service Users’ complex needs.

3) Assist in ensuring that each service user has their needs met through effective assessment and care planning.

Review and monitor care plans.

4) Support and participate in service users’ social and recreational activities in line with service users support plans, including day trips and holidays.

5) Monitor all staff records and support staff to develop positive

working relationships with service users.

6) Promote choice and involvement

7) Ensure all staff members work in a way that demonstrates an understanding and commitment to the rights of people in service
