About The Role

Following the successes of our three Social Prescribers after their first year in post, Ely PCNs wish to recruit more Social Prescribing Link Workers to support their six GP practices across the East Cambridgeshire region.

The principle of Social Prescribing is to empower people to take control of their health and wellbeing and support the communities in which they live.

There are three main parts to this role: direct work with patients, supporting community networks, and providing information to the PCN Board to influence local primary care strategy.

Social Prescribing Link Workers are able to offer individuals time and information, and will take a holistic, strengths-based approach to the person’s health and wellbeing. They are in a position to help the person focus on what matters to them, in the context of their local communities. This approach brings resilience to both individuals and communities.

Link Workers will help connect people to community groups and statutory services for practical and emotional support, and work collaboratively with community partners to support existing and new groups to be accessible and sustainable.

Over the last year, Social Prescribing Link Workers have become a key part of the multi-disciplinary team within the PCN and member- GP Practices.

Full-time or part time hours are available, but ideally we are looking for someone who can offer a minimum 25 hours a week.

About The Candidate

You will be enthusiastic about supporting individuals and communities, and committed to equality, social inclusion, and strengths-based, person-centred approaches. The successful candidate will be able to demonstrate some experience in this area.

The post-holder will need to have excellent communication skills and be comfortable using IT. Building relationships, problem-solving, and person-focus are essential to the success of this role.

Ely Primary Care Networks

Primary Care Networks formed in 2019 to support the join-up of Primary Care GP services in local clusters across England. Ely North PCN and Ely South PCNs cover the East Cambridgeshire region. They work closely with one another and have a joint Board. Together they are known as “Ely Primary Care Networks”. Ely PCNs have had considerable success so far, and are considered a forward-thinking vanguard PCN.

  • £26,000 per annum (pro rata)
