Position: Field Based Care Manager - Hampshire

Location: Winchester, Hampshire

Salary: £25,000 per annum

This person will be covering Hampshire. This role requires someone who is confident at business development and have sense of urgency and enjoyment of gaining new business. A background in a field-based role would be ideal.

  • This is a field-based care manager role.
  • Provide appropriate supervision, training, observations and support to carers
  • To visit and carry out assessments and reviews of client’s needs to produce appropriate support plans
  • To promote a professional image of the company in dealing with new care packages
  • To facilitate local service delivery and manage packages of care

The Care Manager will carry out the following duties:

  • This is a field-based role
  • Carrying out visits to client’s homes and hospitals/other settings to carry out initial client assessments
  • Risk Assessments
  • Start-up of new packages of care with ongoing monitoring and review
  • Client satisfaction reviews
  • Care worker spot checks, supervisions, observations and support
  • Care worker training and competency checks
  • Liaison with other agencies (e.g.: social services) regarding service provision
  • Participation in the on - call system as required
  • Any other duties as reasonably required by the Registered Business Manager or a Director

Job Knowledge, skills and experience:

  • Be an experienced care worker
  • Have good communication skills
  • Be a trained Risk Assessor
  • Have mentoring skills for staff supervision
  • Be able to represent the company at reviews
  • Reliable, flexible and able to respect confidentiality
  • Able to work alone and as part of a team

Competence:

  • To be able to manage and plan workload effectively
  • To be able to establish and maintain effective working relationships
  • To be able to create and maintain administrative systems
  • To be able to take responsibility
  • To have a thorough working knowledge of computers
  • To have good communication skills
  • To be able to work under pressure and to meet deadlines calmly.

Additional information:

  • Full driving licence and own car
  • Willingness to travel to clients’ homes and other sites within the group
  • Subject to DBS check and references
