Care Manager, Adults Team, North West London, up to £30ph

If you are a Qualified Social Worker with experience working with adults and older clients, this could be the ideal role for you.

You will work within a triage team (18+) based a local authority in North West London.

You will be dealing with all new referrals into the council including Mental Health, Older People and Learning and Physical Disabilities.

Main duties will include:

  • Duty Work
  • Safeguarding experience essential as the team manage all safeguarding concerns.
  • Re-ablement assessments- telephone and face to face
  • Crisis Management

Essential requirements:

  • A social work qualification (degree in social work, DipSW, CQSW or equivalent recognised social work qualification
  • HCPC registration as a Social Worker
  • Protocol experience
  • Good communication skills

You will be required to fully register with Remedy for all locum positions.
