Hooray are proud to be supporting a local Hospice to assist in in the recruitment for a Specialist Nurse to join their team on a full or part-time permanent basis.

Hours 
37.5 or 22.5 hours per week Monday - Friday 9am - 5pm 
Plus a requirement to be on call 1 day a week 5pm-10pm/7am-8:30am and 1 in 4 weekends)

As a Specialist Nurse, you will:
-    Coordinate the day-to-day activities of the Hospice at Home Service. 
-    Accepting and processing referrals, ensuring appropriate and timely services are delivered. 
-    Providing leadership, support and guidance to admin, care and external agency workers. 
-    Recruitment, induction, and probation of new HCA'a and RN's and conducting performance reviews. 
-    Support the mentoring process for new staff. 
-    Liaise with GP's, external nurses, hospital teams, and other external agencies to provide the highest level of care.
-    Provide clinical advice and support in the planning and booking of care. 
-    Contribute to the creation of policies, guidelines and protocols and support the governance structures. 

To be successful as a Specialist Nurse, you will need:
-    Registered Nurse with Nursing or Midwifery Council with experience in relevant speciality. 
-    Knowledge of patient services in palliative care.
-    Proven experience within health/social care environment and recent experience of working within a community setting. 
-    Line management experience and in coaching and mentoring.
-    Understanding of clinical governance. 
-    Experience in use of SystmOne 
-    Experience of writing briefing notes and reports. 
-    Desirable: Evidence of clinical specialist qualification in palliative care or oncology.
