Job Description

Clackmannanshire and Stirling Health & Social Care Partnership are looking to appoint an experienced Team Adviser to work in the new Neighbourhood Care team. The Neighbourhood Care Team Adviser will support the team to implement a new multi-disciplinary approach to the assessment, co-ordination and delivery of health and social care to adults in rural southwest Stirling. The Adviser role will be critical to the success of the self-managing team.

The post holder will be experienced in using coaching methodologies, and will work to embed the principles of the neighbourhood model of care in the team. The Adviser will develop and coach the team in their decision making processes and provide support with problem solving, relationship building & resolution of complex issues. Interpersonal skills are very important; the adviser needs to be able to put people at ease and build strong working relationships with the team and wider services. The post holder will have refined analytical skills and the ability to effectively assess the team, identify issues and provide effective support and mentoring.

The post will be based within the Rural South West of Stirling across a number of care hubs in the locality. This post is Part Time for 1 Year from date of appointment.
