If you are nurse or allied health professional with experience in care planning and co-ordination of care for a patients with long term conditions including MND patients and you have an interest in this specialist area, we want to hear from you.

MND Co-ordinator will work autonomously, but will require to collaborate closely with a wider teams such as MND Association, rehabilitation team, Palliative Care team, Speech and Language therapy team and neuro- centres in Bart's Health and BHRUT hospitals.

MND Co-Ordinator will work as part of the integrated services in Redbridge, collaborating with wide range of professionals such as nurses, social workers, occupational therapist.

Diagnosis of MND affect people physical and mental wellbeing as well as their families. You will be making a difference to people lives as a 1st port of contact for care planning, care co-ordination and support.

MND Co-ordinator role offer 18.45 hours of work; we welcome applicants who would like to job share with other position.

You will be subject to 6 months probation during which you will be equipped with relevant support in form of induction, supervision, training, shadowing, networking and admin support.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £3,722 to a maximum of £4,743).
