Medicines Optimisation Pharmacist

An exciting opportunity has arisen to lead some key medicine optimisation projects as part of the NHS North of England Commissioning Support (NECS) Medicines Optimisation Team in the Durham and Tees area. We are looking for a dynamic pharmacist to join our well regarded, highly skilled and experienced medicines optimisation team.

The Medicines Optimisation team is part of the NECS extensive commissioning support operations providing medicines optimisation, commissioning and clinical services across the North of England and Yorkshire. The team has a track record of delivering high quality Medicines Optimisation services for many years. The achievements of the team are based on the collaboration of all team members whilst recognising and embracing the individual skills of each team member. We havea strong focus on the principles of medicines optimisation, a patient centred approach by focusing on individual values, needs and priorities to get the best from investment in and use of medicines.

The post holder will be responsible for developing and leading the implementation of medicines optimisation projects which support the medicines optimisation strategy within the assigned Clinical Commissioning Group. You will work in collaboration with other pharmacists and technicians across our organisation to continually improve the way in which we deliver services.

The candidate should demonstrate excellent clinical, leadership, communication and negotiation skills and have experience of implementing medicines optimisation, quality and patient safety initiatives and resolving prescribing issues. You will be a member of the General Pharmaceutical Council and must also be computer literate, organised and be able to work independently as well as part of a team. The successful candidate will be able to prioritise workload, lead and develop a team, build key relationships and be confident interacting with a wide range of stakeholders such as GPs, practice nurses, secondary care clinicians and patients.

NECS is one of the leading commissioning support organisations in the country, with a successful and experienced medicines optimisation team. The Medicines Optimisation service supports Clinical Commissioning Groups in meeting their statutory responsibilities in commissioning medicines in accordance with medicines related legislation. The team assists the CCG in meeting with their obligations to patients under the NHS Constitution, with respect to local decision making around medicines-related commissioning. We have a strong belief in the patient centred principles of medicines optimisation, focusing on individual values, needs and priorities to get the best from the investment and use of medicines. You will need an ability to establish and maintain effective internal and external working relationships and networks with patients, care home staff, the CCG, PCNs, GPs, community pharmacies and care home managers.
