Position: Team Manager - Social Worker (2 vacancies)

Our Leaving Care Teams play a vital role in empowering care leavers to achieve their goals and aspirations. As the Team Manager, you'll be part of a dedicated community that values inclusivity and resilience, working together to make a positive impact on the lives of care leavers.
The Role: In this role, you'll lead and inspire a team of social workers and personal assistants in providing essential guidance and support to care leavers. Your responsibilities will include:
- Developing and implementing comprehensive support plans to facilitate a smooth transition to independence.
- Collaborating with external agencies and support services to ensure holistic support for care leavers.
- Championing innovation and best practice to enhance outcomes for care leavers.
