Role Description:
As an Adult Support Worker, you will play a pivotal role in providing person-centered care and support to adults in need within the Perth area. Your responsibilities will include:

Building strong and trusting relationships with individuals, understanding their unique needs, and promoting their well-being.
Assisting individuals in their daily activities, including personal care, household tasks, and medication management.
Supporting individuals in accessing community resources, social activities, and educational opportunities.
Collaborating with a multidisciplinary team, including healthcare professionals and social workers, to develop and implement tailored care plans.
Actively listening to individuals' concerns, providing emotional support, and advocating for their rights and preferences.
Maintaining accurate and timely documentation of care provided and participating in team meetings and training sessions.


Requirements:
To excel in this role, we are looking for candidates who possess the following qualifications and skills:

Previous experience in a similar role, providing care and support to adults.
Empathy, compassion, and a genuine passion for improving the lives of others.
Excellent communication skills and the ability to build rapport with individuals, their families, and the wider support network.
Flexibility to work on a rotational shift basis, including weekends and evenings.
Basic computer literacy and proficiency in maintaining accurate records.
NVQ/QCF Level 2 or 3 in Health and Social Care (desirable but not essential).
