Specialist Assessment Co-ordinator: Community Mental Health Team
Location: Oldham
Band 6
Hours: 37.5 hours per week, Monday-Friday
Main Duties & Responsibilities:
- The role is to work as assessment and/or care coordination worker for referrals received into the team from other teams, i.e. HTT, Inpatient Wards and Access.
- To act as a secondary care assessment worker/short term intevention, carrying out assessments of clients referred to CMHT as they are suffering with a severe and enduring mental illness requiring secondary care. To support and supervise junior staff within the team. To at times manage and supervise the team.
- To support the assessment of the needs of service users by utilising specialist research validated assessment tools
- To support the risk assessment process through development of and application of skills in identification of risk, implementation of and evaluation of risk management plans.
- You will be expected to provide a full holistic assessment and risk assessment, with the aim to establish if the person referred requires longer term support from the CMHT which would result in transfer to care co-ordination, or alternatively to identify short term intervention which will be provided within the assessment function of the team.
