The Care for Children and Care Experienced Team are looking to appoint a dedicated and committed Social Worker, to join a service that is passionate about our children and care experienced young people.

Building positive, meaningful relationships, promoting permanence, undertaking direct work and making a positive impact in the lives of our truly remarkable children and young people, their families and those who are important to them?

If so, an exciting opportunity has arisen to join our Cared for and Care Experienced Team.

Duties:

Assessment: Social workers conduct assessments of the needs and circumstances of individuals who are under the care of the team. This involves gathering information from various sources, including the individual, their family members, and other professionals involved in their care.

Developing Care Plans: Based on the assessments, social workers develop care plans tailored to the individual's needs and circumstances. These plans outline the services and support that will be provided to help the individual achieve their goals and improve their well-being.

Case Management: Social workers coordinate the implementation of care plans, ensuring that the necessary services and support are provided to the individual. This may involve working with other professionals, such as health care providers, educators, and support workers.

Advocacy: Social workers advocate on behalf of the individuals they serve, ensuring that their rights are protected and their voices are heard. This may involve representing them in meetings with other professionals or advocating for changes to policies and procedures that affect their well-being.

Crisis Intervention: Social workers provide crisis intervention and support to individuals who are experiencing crises or emergencies. This may involve assessing risk, providing immediate support, and coordinating with other services, such as emergency housing or mental health support.

Support and Counselling: Social workers provide emotional support and counselling to individuals under their care, helping them to cope with challenges and develop strategies for managing their situations.

Collaboration and Networking: Social workers collaborate with other professionals and organisations to ensure that individuals receive comprehensive and coordinated care. This may involve attending meetings, participating in case conferences, and building relationships with other service providers.

Documentation and Reporting: Social workers maintain accurate and up-to-date records of their interactions with individuals under their care, as well as any services provided or referrals made. They also prepare reports for their organisation or relevant authorities as required.

Training and Education: Social workers may provide training and education to individuals under their care, as well as their families and caregivers, on topics such as life skills, health promotion, and navigating systems and services.
