Responsibilities:

  • Conduct holistic assessments of adults experiencing mental health difficulties, considering their social, emotional, and practical needs.
  • Develop and implement person-centered care plans in collaboration with service users, promoting recovery, resilience, and independence.
  • Provide therapeutic interventions and support services to help adults manage symptoms, improve coping strategies, and enhance their quality of life.
  • Work collaboratively with mental health professionals, including psychiatrists, psychologists, and community mental health teams, to ensure coordinated and integrated care.
  • Advocate for the rights and preferences of service users, ensuring they have access to appropriate support, resources, and opportunities for inclusion and participation.
  • Monitor and review care plans regularly, adapting interventions as needed to reflect changes in the service user's circumstances and goals.
  • Facilitate access to community resources, housing, employment, education, and other support services to promote holistic recovery and social inclusion.

Requirements:

  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working with adults experiencing mental health issues, preferably within a statutory mental health setting.
  • Sound knowledge of mental health legislation, policies, and best practices related to adult mental health care.
  • Strong assessment, communication, and interpersonal skills, with the ability to build rapport and trust with individuals from diverse backgrounds.
  • Ability to work collaboratively within multidisciplinary teams and across different agencies to achieve positive outcomes for service users.
  • Compassion, empathy, and a non-judgmental approach to supporting individuals experiencing mental health challenges.
  • Commitment to continuous professional development and adherence to ethical standards and professional boundaries.
