Adults Social Worker Level 3 - Band J

Here at Randstad we have an exciting opportunity available for a qualified Adult Social Worker (Level 3) within a Rotherham based learning disabilities team.

Our client is looking for enthusiastic and passionate individuals who care about getting the best outcomes for people with a Learning Disability / Autism.
We welcome applications from those who are not afraid to challenge and who will promote independence and autonomy using a strength based, person centred approach. This is a newly established permanent Learning Disability team
working with other professionals, organisations /providers, carers and families.

Responsibilities:

  • Conduct in-depth assessments of adults social care needs, considering physical, mental health, cognitive, environmental, and socio-economic factors.
  • Utilise advanced assessment tools and methodologies to gather information and formulate holistic care plans tailored to individual strengths, preferences, and goals.
  • Lead multi-agency assessments and contribute expertise to complex cases, ensuring a thorough understanding of clients needs and risks.
  • Provide expert advice, guidance, and specialist interventions to address complex social, emotional, and practical challenges faced by adults and their families.
  • Offer advanced therapeutic support and counselling to individuals experiencing mental health issues, trauma, loss, or significant life transitions.
  • Develop innovative and evidence-based interventions to promote resilience, coping skills, and social inclusion among vulnerable adults.
  • Lead safeguarding investigations and coordinate multi-agency responses to safeguarding concerns, ensuring timely and robust risk assessments and interventions.
  • Provide expert testimony and evidence in safeguarding conferences, case reviews, and court proceedings, advocating for the best interests of vulnerable adults.
  • Develop and implement risk management plans in collaboration with clients, families, and relevant professionals to minimise risks and enhance safety.
  • Coordinate complex care packages and support arrangements for adults with multiple and complex needs, involving input from health, social care, housing, and voluntary sector partners.
  • Facilitate multi-agency meetings, case conferences, and care reviews to promote integrated and person-centred care planning and decision- making.
  • Act as a key liaison between clients, families, carers, and service providers, ensuring effective communication, collaboration, and continuity of care.
  • Provide clinical supervision, mentoring, and support to less experienced
  • social workers and multidisciplinary team members, sharing expertise and
  • promoting best practice.
  • Engage in continuous professional development, staying abreast of advances in social work practice, legislation, and research relevant to adult social care.
  • Contribute to the development of policies, procedures, and practice standards within the organisation, promoting a culture of excellence and nnovation.
  • Qualifications:
  • Bachelors or Masters degree in Social Work or a related field.
  • Registration as a Social Worker with the relevant regulatory body.
