As a SCA you would work alongside members of the wider multi-disciplinary team (MDT), which includes psychiatrists, CMHNs, health nurses, occupational therapists, GP

We require a SCA who, ideally, have a background in adult social care, but who also possess a varied range of transferable skills and experience.

Able to complete Care Act Assessments, risk assessments and asset-based support planning.
Undertaking reviews
Experienced in handling a lively case load and be able to work autonomously and within a multi-disciplinary team.

The successful candidate will report to a Team Leader/Community Manager.
