Service Care Solutions are recruiting on behalf of Liverpool City Council for a Social Care Assessor to join their Adults Service.

Job Description

The successful applicant will be required to complete Care Act assessments, risk assessments and asset-based support planning. They will also be required to under-take reviews. They will also work alongside members of the wider multi-disciplinary team (MDT), which includes psychiatrists, CMHNs, health nurses, occupational therapists and GP's.

Requirements

  • Experience in handling a lively case load and the ability to work autonomously/ within a multi-disciplinary team.
  • Full enhanced DBS
  • Have a background in Adults Social Care

Full time position - 35 hours per week - £19.40 per hour.
