Job Title: Senior Social Worker - North Transfer of Care Team

Location: Hinchingbrooke Hospital, with occasional travel to Peterborough City Hospital and Queen Elizabeth Hospital Kings Lynn

Position Overview: We are seeking a dedicated and experienced Senior Social Worker to join our North Transfer of Care Team. As a Senior Social Worker, you will be responsible for providing comprehensive social work assessments and interventions to support patients and their families during the discharge process. While the role is primarily based at Hinchingbrooke Hospital, there may be occasional travel required to Peterborough City Hospital and Queen Elizabeth Hospital Kings Lynn.

Responsibilities:

  • Conduct thorough social work assessments to identify patient's needs and develop appropriate discharge plans.
  • Collaborate with multidisciplinary teams to coordinate discharge planning and ensure continuity of care for patients.
  • Provide emotional support and counselling to patients and their families, addressing concerns and facilitating coping strategies.
  • Liaise with community resources, social services, and other agencies to arrange post-discharge support and services for patients.
  • Advocate for patient's rights and preferences, ensuring their voices are heard in the discharge planning process.
  • Maintain accurate and timely documentation of social work interventions and discharge plans in accordance with hospital policies and procedures.
  • Participate in team meetings, case conferences, and training sessions as required.

Requirements:

  • Bachelor's or Master's degree in Social Work or a related field.
  • Current registration with the relevant professional body (e.g., SWE).
  • Minimum of 3 years of post-qualifying experience in social work, preferably in a healthcare setting.
  • Strong assessment and intervention skills, with the ability to work effectively with diverse patient populations.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with patients and their families.
  • Ability to work independently and collaboratively within a multidisciplinary team environment.
  • Flexibility to adapt to changing priorities and case-loads.
  • Proficiency in Microsoft Office and electronic medical records systems.
