Job Title: Adults Social Work - Senior Practitioner - LD Community Team
Location: Haringey
Salary / Pay Rate: £35 per hour umbrella
Job Type: Contract (3 months+)

The Role

This role is required to cover a split funded post within our Assessment and Intervention Team (AIT). A part of the service that will sit outside of the locality model currently.

The role is a Senior Practitioner role required to support with the cases known to AIT. This will include looking to:
- prevent or reduce the risk of hospitalisation
- readmission to hospital
- support successful discharge from hospital

Where hospital is referenced above, this is referring to Mental Health Admission to hospital. The person in the role will usually be working with people who have been under s.2 or s.3 of the Mental Health Act.

The Candidate

• Experience supervising a team of Social Workers
• Experience working with people who have been sectioned in the community
• Experience of working with people with Learning Difficulties.
• Registered with SWE
