Key Highlights:

• Complete Care Act assessments, support plans and reviews of package of care and care home placements.
• Navigate cases in accordance with Care Act, Mental Capacity Act, and Human Rights Act.
• Build meaningful connections with individuals, families, and the community.
• To work on duty to screen new referrals and respond to telephone calls.

Necessary Requirements (Your application will not be considered if you lack all three):

• Degree in social work
• Up to date Social Work England registration
• Significant front line experience in adult services within the UK
