Referral & Assessment Social Worker

My client is currently seeking a compassionate and skilled Social Worker to join our team as a Referral and Assessment Social Worker.

In this role, you will be responsible for conducting thorough assessments of individuals and families in need, as well as connecting them with appropriate social services and resources. This position requires strong clinical skills, empathy, and a commitment to social justice.

Key Responsibilities:

Receive referrals from various sources, including community agencies, health care providers, and individuals seeking assistance.
Conduct comprehensive assessments to evaluate the needs, strengths, and risks of clients.
Collaborate with clients to develop individualized service plans tailored to their specific needs and goals.
Provide information and referrals to clients regarding available resources, programs, and support services.
Advocate on behalf of clients to ensure they have access to necessary services and resources.
Maintain accurate and up-to-date documentation of client assessments, service plans, and progress notes.
Collaborate with interdisciplinary team members to coordinate services and support for clients.
Participate in regular supervision, team meetings, and professional development activities.

You must have a degree in social work, be on the SWE register and have extensive post qualification experience to be shortlist for this role.
