Reablement Practitioner- Barnsley

Are you a passionate, dedicated Reablement Practitioner looking for your next role? 4Recruitment Services are recruiting a Reablement Practitioner to join a team based in Barnsley.

Umbrella Pay- £16.81 per hour

05/02/2024 – 3 months initially

Monday to Friday, 37 hrs/week – Hybrid

The role:

  • As a Reablement Practitioner you will help to support and improve the lives and wellbeing of the people in Barnsley, helping people regain and develop new skills so that they can return home and maintain their independence You’ll have a life-changing impact.
  • The service delivers a broad range of support to enable people to regain and maintain their independence and daily living skills within their home environment. We are looking for people who want to help people of all ages remain at home and get their life back on track following episodes of illness or an event that has affected their life skills.
  • You will be required to undertake strength- based assessment which places the service user at the centre of the decision-making process and engages them and their circle of support in agreeing goals and facilitating outcomes which maximise their independence.
  • To review and revise support plans at agreed intervals throughout the pathway, completing a final outcome report for each service user and as appropriate identify any ongoing support needs making recommendations as to the level and type of any future service provision.
  • Ensure a person-centred holistic approach, which includes activities of daily living and social
    connectedness, facilitating opportunity to participate in community activity, aiming to reduce potential social isolation and positively influence overall wellbeing.
  • To work in partnership with a number of internal and external stakeholders ensuring the service users journey is consistent, safe and reflects desired outcomes and goals.
  • Participate in the day-to-day organisation of workload, including staff allocation via electronic systems.
  • To act as ‘Duty’ officer on a rota basis by undertaking Triage of new cases and being initial point of contact into service.

Requirements

  • UK Driving Licence, access to a vehicle, business insurance
  • Relevant Level 2/3 qualification
  • Experience of working with vulnerable groups
  • Significant understanding of the social care needs of older people and vulnerable adults
  • Good knowledge of care assessment and care management processes
  • Experience building collaborative working relationships with user’s support network and partner agencies
  • Enhanced Adult DBS registered to the update service
