Randstad are recruiting for a Qualified Social Worker to join Liverpool City Council's Discharges from Community Intermediate Care Centre . This is a full time role, offering an amazing pay rate of £33 per hour.

Responsibilities:

• Discharges from Community Intermediate Care Centre (CICC) based at Clatterbridge Hospital.
• Working in an MDT setting to improve adult patients and their carers experiences, avoid hospital admission and reduce long length of stays.
• Assessing people who have been placed on a discharge to assess (D2A) pathway from hospital into nursing homes.
• Work with adults and their carers to provide access to information, advice and services including support and access to information to enable self-care and self-management.
• Assess long term care needs via the 3 conversation model and develop support plans to enable people to return to their own homes where appropriate or another community setting.
• Where a person may not have capacity for a decision about their support plan/discharge arrangements, apply the Mental Capacity Act 2005 informed by the MCA Code of Practice and relevant case law.

Requirements:

• Social Work Degree or equivalent
• Social Work England registration
• Enhanced DBS
