We are looking to recruit a Qualified Social Worker in the Hospital Discharge Team within the Social Care Industry. Our client has a well-structure and organised team that are looking to grow their staffing levels. The role offers has a supportive team culture with regular supervision on cases. This is an exciting opportunity to join a fast-paced and supportive team within Wandsworth Council.

Main duties include:

  • Working with Vulnerable Adults within a hospital setting
  • Report writing and making assessments
  • Developing care plans
  • Care management
  • Managing complex cases
  • Liaising with other professional bodies
  • Arranging safe and timely discharges for service users
  • Self-directed support / personal budgets

To be successful:

For this role you will need to be a Qualified Social Worker with a SWE Registration and educated to Degree Level IE: BA SW, BSc SW, DIPSW. The client requests that the worker will have significant Court Experience and can work with a full caseload. You will also need to have the Eligibility to Work in the UK with previous experience as an Adult Social Care
