We have a fantastic Locum Social Work Opportunity based within a Continuing Healthcare Team in the Burnley, Lancashire area. The role will be focusing on NHS Continuing Healthcare reviews.

Responsibilities:

• Identifying what care falls within the responsibility of the local authority
• Work within an MCDT approach, undertaking social care assessments
• Undertaking decision support tools for individuals who require a review of CHC eligibility

To be considered for the Social Worker role:

• 1-3 years experience of working within a similar social work setting
• Diploma/Degree in Social Work
• Registration with Social Work England
• Liquid Logic Experience
• Knowledge of the National Framework for NHS Continuing Healthcare and The Care Act (2014).
