Purpose of the job

To be responsible for the implementation and continuation of physiotherapy programmes identified in customer person centred plans (PCPs).
Responsible for liaising with other professional agencies in relation to the provision of physiotherapy services. Line manage therapy assistants (where applicable).

Key responsibilities

* Provide an appropriate programme of physiotherapy for existing and new customers and encourage active participation in therapy sessions.

* Provide a holistic and objective assessment of physiotherapy needs for all customers.

* Maintain and further the independence of customers by use of appropriate physiotherapy.

* To be involved in the admission process of each customer, contributing to their PCPs, as well as attending annual reviews and provision of reports, as and when necessary.

* Liaise with the respective Primary Health Care service (such as Community Physiotherapist and/or Occupational Therapist) in the in the assessment and provision of specialist equipment.

* Liaise with appropriate staff regarding physiotherapy issues and recommendations to ensure the best continuity of support for customers in line with their PCPs.

* Ensure that all physiotherapy equipment and facilities (including therapy pools where present) are maintained in accordance with manufacturers' instructions and organisational procedure.

* Provide education and training for staff, volunteers, customers and family, as appropriate, including advising on safe moving, handling and transfer techniques and assisting with risk assessments.
* Support and facilitate customer communication with their relatives, friends, professionals and other members of staff, in order to ensure the continuity of quality and safety in the provision of support.

* Maintain comprehensive and confidential physiotherapy records, including recording any recommendations that will involve support staff participation.

* Liaise with social workers, health authorities, Social Services, doctors, nurses, etc, where appropriate.

* Work within professional code of conduct and organisational policy and guidance.

* Facilitate the continuing development of physiotherapy provision at the service, seeking assistance from Leonard Cheshire and other colleagues, internally or externally.

* Comply with all Health and Safety policies and procedures.

* Comply with company and any local safeguarding and mental capacity act (MCA) policies and procedures.

* Work within the regulatory compliance requirements applicable at the respective service.

* Maintain an awareness of individual customer risk assessments relating to specific support needs, for example, dysphagia, mobility and safety.

* Participate in supervision, staff meetings and training activities (including updates) as required, sometimes off-site and outside normal hours.

* Responsible for maintaining and improving own knowledge and skills through experience and training.

* Undertake additional responsibilities as requested by the Service Manager following the successful completion of specific training and personal skills development.

* Undertake any other reasonable duties requested.

Person specification
Essential requirements

* To be qualified and registered as a practicing Physiotherapist with the Health and Care Professions Council (HCPC).
* To be able to work as an autonomous practitioner as a full Member of the Chartered Society of Physiotherapy.
* Thorough and up-to-date knowledge of physiotherapy theory and best practice, being able to demonstrate effective physiotherapy practice in all qualified procedures.
* Previous experience in a range of clinical skills with an emphasis on neurological rehabilitation and the management of disability.
* Experience of working within a multi-disciplinary team and communicating with professionals and other agencies.
* Able to work flexibly in accordance with the needs of the service.
* Some prior experience of managing and supervising other staff would be advantageous.
* Clear driving licence (if driving is required).
* To have a genuine commitment to the values and ethos of the organisation

* Demonstrate awareness of evidence-based practice.
* Demonstrate sound observational skills to identify potential or actual changes in health status of customers.
* To have good verbal and written communication skills in English and a good level of numeracy so as to be able to maintain accurate records.
* Computer literate, able to use standard Microsoft packages.
* To be discreet and able to maintain confidentiality.
* Ability to train, coach and develop staff.
* Knowledge of Service User self-advocacy and empowerment.
* Excellent interpersonal skills.
* Ability to work within a prescribed budget.
