We are seeking compassionate and dedicated individuals to join our team as Home care Mobile Responders. In this role, you will have the opportunity to work closely with a colleague to provide high levels of care to individuals who do not have access to regular care in their own homes. Your primary responsibility will be to support people in need and help reduce unplanned hospital or care home admissions by delivering personalised care and support services.

Key Responsibilities:

  • Work collaboratively with a colleague as a team to provide compassionate care and support to individuals in their own homes.
  • Assist individuals with activities of daily living, including personal care, medication administration, meal preparation, and household tasks.
  • Provide emotional support and companionship to clients, promoting their overall well-being and independence.
  • Assess each person's needs and develop personalised care plans to meet their individual requirements.
  • Communicate effectively with clients, their families, and health care professionals to ensure coordinated care and support.
  • Maintain accurate records of care provided and report any changes in clients' condition to the appropriate authorities.

Requirements:

  • Previous experience in a care or support role is preferred but not essential.
  • Compassionate nature and a genuine desire to make a positive impact in the lives of others.
  • Ability to work effectively as part of a team and independently when required.
  • Excellent communication and interpersonal skills.
  • Flexibility to work various shifts, including evenings and weekends.
  • Must have access to reliable transportation.
