Position Overview: We are currently seeking a highly skilled and experienced Locum Clinical Pharmacist (Band 7) to join our team. In this role, you will be responsible for providing comprehensive pharmaceutical care to patients, collaborating with multidisciplinary teams to optimize medication therapy and improve patient outcomes.

Responsibilities:
  • Conduct medication reviews and assessments to ensure the safe, effective, and appropriate use of medications.
  • Provide clinical pharmacy services, including medication reconciliation, therapeutic drug monitoring, and dosage adjustments.
  • Collaborate with healthcare professionals to develop and implement medication management plans tailored to individual patient needs.
  • Participate in ward rounds, clinics, and multidisciplinary team meetings to contribute expertise on medication-related issues.
  • Deliver patient education and counselling on medication adherence, potential side effects, and lifestyle modifications.
  • Contribute to the development and implementation of pharmacy policies, protocols, and guidelines.
  • Maintain accurate documentation of pharmaceutical interventions, clinical recommendations, and patient interactions.

Requirements:
  • Bachelor's or Master's degree in Pharmacy from a recognized institution
  • Registration with the General Pharmaceutical Council (GPhC) as a Pharmacist
  • Proven experience working as a Clinical Pharmacist in a hospital or clinical setting
  • Strong clinical knowledge and expertise in pharmacotherapy, pharmacokinetics, and pharmacodynamics
  • Excellent communication, interpersonal, and teamwork skills
  • Ability to work autonomously and collaboratively within a multidisciplinary team
  • Commitment to continuous professional development and evidence-based practice
