A Local Authority in the South West England is hiring for a full time Adults' Learning Disabilities Social Worker to adhere to local adults in need that are in use of the support from adult's social care as part of the community and assessment teams.

Key Duties include:

• Screen all referrals coming in to the team
• Complete duty visits
• Care Act assessments and Mental Capacity Assessments
• Prevent delays and ensure care is not a delay for external partners

What are they looking for?

• Qualified social worker with 1 year + adult learning disabilities experience (expected)
• Registered with SWE (expected)
• Professional qualification in Social Work (expected)
