We're recruiting for a temporary (6 months) lead complex case managers (Band 7) on behalf of our client based in Birmingham

The individual in this role will conduct thorough assessments for adults over 18 with complex health needs, including review and monitoring of care funded through NHS Continuing Healthcare. This encompasses retrospective and appeal cases. Acting as a Case Manager, they will assess clients' needs and work collaboratively with stakeholders to plan and evaluate service provision, ensuring optimal care in the most suitable environment. They will also liaise with colleagues across health, social care, and the independent sector to ensure appropriate care management processes, potentially presenting assessments to BSOL continuing care panels for approval.

Requirements:

Essential:

- Educated to degree level or equivalent experience in a specialist area

- Dual registration as LD Nurse/Registered Nurse with a Master's in healthcare or equivalent expertise

- Current NMC registration or equivalent qualification/experience

- Previous experience in Continuing Care Assessments

- Significant experience with adults and young people with complex health needs

- Experience negotiating and liaising with multidisciplinary teams

- Evidence of audit involvement and outcome implementation

- Ability to demonstrate autonomous practice and initiate evidence-based practice

- Commitment to partnership working across education, health, and local authority settings

- Negotiation, conflict management, leadership, and team development skills

- Understanding of budget management and financial processes

- Knowledge acquired through CPD

- Working knowledge of DOLS, penalisation, and SEND agenda

- Familiarity with NHS Continuing Health care/NHS Funded Care and national tools

- Understanding of safeguarding, transition processes, equality, diversity, and patient experience agenda

- Knowledge of Mental Health Act 1983 and Care Act assessment

- Ability to investigate complaints and present to courts

- Working knowledge of Who Pays Guidance 2020, operational management, and NHS role

- Understanding of Social Services, Local Authority functions, and primary care

- Highly developed communication skills, including presenting complex cases

- Ability to work across boundaries and build effective working relationships

Desirable:

- Reflective supervision, mentoring, facilitation, coaching, and clinical supervision skills

- Ability to hold oneself and others accountable for delivering better outcomes

- Inclusivity, supportiveness, and empowerment of others

- Engagement and enablement of collective aims

- Extensive nursing skills in complex healthcare needs

- Excellent report writing and interpersonal/communication skills

- Sensitive management of client and family/carer expectations

- Effective workload management and decision-making in tight timelines
