Homelessness Service Co-ordinator

£24,700 a year

Stoke-on-Trent

Permanent, Full Time

Our client looking for a Service Coordinator to deliver high quality housing and support to customers who have complex needs and experience multiple disadvantage. You’ll coordinate customer access to a wide range of services and lead on the design and delivery of a service coordination plan.

This role is a mixture of working out in the community with customers, at their Stoke office and from home.

Job requirements

- Work with customers to identify their support needs and address any wider issues associated with homelessness. You’ll provide support either in their current home or help them to secure a home of their own.

- Empower customers to regain independence and help them understand what it takes to manage their own tenancy.

- Build and maintain positive relationships with other of our clients services and external partners to increase the level of support available to customers. This includes attending regular internal and multiagency meetings.

- Make sure all information is recorded, collated, updated and saved securely to meet contract, service and organisation requirements and standards.

- Work closely with the Support Team Leader to promote their services, build positive relationships with the local community and help monitor and evaluate the service.

What they’re looking for

- Someone educated to GCSE Level or with a good level of maths and English.

- Someone with experience of supporting adults with complex needs such as substance misuse, homelessness and mental health, or transferable skills you can bring to the role.

- Great communication, problem-solving, organisational and time-management skills.

- Someone with a genuine understanding and compassion for people without a home, who won’t judge but instead will empower customers and give them a voice.

- Someone with a full UK driving license who is able to use their own vehicle for work purposes.

- DBS check.
