Family support worker
Prospero Health & Social Care are recruiting Young Person Support Workers.
About the service:
To provide tailored support for a young person on request by social services, this would be target specific and would be reviewed on a week to week basis. The planned support would be to meet the needs of that young person ie, A 6 week healthy living programme where you would target exercise, diet, fluid intake, hours of sleep etc and monitor it weekly. The outcome of such programmes would be to increase the health and well-being of the young person.
Shifts - Adhoc/flexible
Main duties and responsibilities:
- Working with young people within the family home or out in the community
- Supporting and supervising young people who may present with challenging behaviours
- Produce case reports and correspondence.
- Activities
- Liaise with social services regarding set targets
- Accessing community services
- Working alongside other professional networks
Experience, training, and qualifications:
- Substantial experience of working and engaging with young people
- Capable of dealing with challenging behaviour
- Capable of working well on your own
- A positive attitude

- Have an Enhanced DBS registered on the update service or willing to process an Enhances DBS through Prospero.
- Be able to provide 5 years references.
