Qualified Social Worker - Ealing - Mental Health Team

We are seeking experienced Social Workers to work in the Mental Health Social Care Team for the London Borough of Ealing. The service is made up of a number of teams and is responsible under the Care Act for assessing and reviewing the social care needs of adults within Ealing who have primary mental health needs. This includes creating person centred, strengths based support plans, and undertaking ,capacity assessments under the Mental Capacity Act, Completing, Social Circumstances Tribunal reports and undertaking the role of a Social Supervision for conditionally discharged s37/41 patients .

Successful candidates will have a solid understanding of the legal frameworks of the Care Act (2014),, the Mental Capacity Act 2005), the Mental Health Act1983 (2007) and the Human Rights Act(1998)-, can demonstrate professional curiosity, and an ability to work in a strength based way to ensure the best outcomes for residents Ealing with mental health needs. Experience with the Court of Protection Schedule 21A challenges, will be considered in the recruitment process. Candidates are required to have robust assessment skills and be able to complete their interventions within the team timescales.

The service also has the statutory responsibility for undertaking safeguarding enquires for adults with mental health needs. In the role you will be acting as a Safeguarding Enquiry Officer, working closely with a Safeguarding Adults Manager to make enquiries, and manage risk, ensuring people's individual safeguarding outcomes are met.

The service also provides a duty system; which Social Workers contribute to on a rota basis. Duty is responsible for screening incoming referrals to the service, screening safeguarding concerns, responding to queries and contact from service users, carers and other professionals.

The mental health social work team is currently, co- located with West London NHS Trust. We're looking for social workers to work the across the borough. The team operates in a robustly planned multidisciplinary framework.

Driving is desirable but not essential.

Your main responsibilities include:

* Hold and effectively manage a complex and varied caseload with appropriate supervision, guidance, and support, reflecting the function of the team and the Vulnerable People Service in accordance with policy, procedures, guidance and legislation.
* Undertake work within legislative frameworks including the preparation of written statements and representations to the judiciary at Court hearings.
* You will be accountable for complex decision making in relation to caseloads ensuring professional judgement, service user involvement, needs led assessment, critical reflection, and analysis to inform this.
* You will identify and assess levels of risk and need (within statutory frameworks) in often complex situations. Undertake safeguarding investigations and deliver protective and/or supportive services for individuals at risk and maintain up to date assessments, care plans and reviews.
* You will be preparing statements of need/care and support plans, in consultation with other agencies as appropriate, that reflect the wishes of the individual and lead to the commissioning of personalised packages of care.
* You will undertake direct work with adults and their families in line with care plans.
* You will work co-operatively with both internal and external colleagues across multi - agency boundaries.

Essential requirements:

* Social Work England registration
* Eligible to work in the UK
* 2 years+ post-qualified experience
* Degree level or equivalent in Social Work
