Medacs Helathcare are recruiting for a Discharge Senior Nurse in St Mary's Hospital.

Qualified RN/PT or OT qualification

Shift times 8 - 6pm but flexible and will be 7 day rota with weekends included.

  • Knowledge of community services, hospital discharge processes, excellent clinical risk assessment
  • knowledge of nursing and functional community equipment, knowledge of referral pathways.
  • Experience of providing clinical supervision & student training

Excellent communication skills and efficient time management, able to manage busy caseload, excellent computer skills

If you know a lot about hospital discharges and are able to problem solve and work closely with the MDT with in the hospital then this role would suit you.

skills needed :

1) Knowledge of community services, Knowledge of hospital discharge processes, excellent clinical risk assessment.

2) knowledge of nursing and functional community equipment, knowledge of referral pathways

3) Excellent communication Skills and efficient time management, To be able to manage a busy caseload, computer skills.

4) Attend the first floor meetings 8.30am and board round at 12 - this is to cover the first floor

5) To log in to Cerner daily and follow up on the patients - awaiting care packages Ie identify the patients that can be discharged once MFFD

6) Daily meetings on Teams

7) To be able to communicate and hand over to the community Home first team Daily

8) To be able to follow up with the Therapist on the wards and

9) Documentation on the patient's hospital notes

10) Identifying any issues or raised or concerns with the patients NBA
