JOB SUMMARY / PURPOSE:

  • The post holder will ensure the comprehensive provision of a cost effective, accessible, appropriate and responsive Community Mental Health Team (CMHT) service to service users with complex mental health needs.
  • The post holder will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.
  • The Manager will develop and coordinate a multi-professional approach providing evidence based mental health care to all those who use the service.
  • This collaboration will harness an integrative agency approach so that physical, psychological and social care agencies are working together in providing holistic care to individuals who require its support.
  • The post holder will be responsible for the continued development of the CRHT and will ensure service delivery is of a high professional standard, in line with Health Board policies and procedures.

DUTIES & RESPONSBILITIES

  • To work within the policies, procedures and guidelines set out by the Health Board and professional bodies. Operate within the guidelines set by the NSF, Clinical Governance, Risk Management and NICE guidelines.
  • Establish and maintain effective care through the holistic and comprehensive assessment of service user’s needs, planning, implementation, and review in accordance with CTP policy and procedures.
  • To complete comprehensive risk assessment and risk management plans in accordance with Health Board policy and guidance.
  • To ensure multi-disciplinary working by utilising other professionals to enhance care and provide information and feedback for multi-disciplinary review and referral meetings.
  • To promote a value base service which meets the needs of service users, promoting active engagement, developing life skills and roles promoting choice, empowerment, independence and ensuring that their wishes are respected.
  • To encourage the participation of user’s and carer’s in the development and review of the service.
  • To ensure that the Team develops services that are sensitive to the individual needs of users and carers taking into account issues of race, gender, sexuality, age and religion.
  • To take responsibility within the team for ensuring the assessment of health and social needs and the formulation, planning, delivery and evaluation of care.
  • To take responsibility for managing adverse incidents, suicide and untoward/unexpected death incidents ensuring that these are recorded, processed and policy implemented.
  • To take responsibility for ensuring that within the team that there is ease of access to the service, clear processes and assessments.
  • To prioritise need/ cases and workload within the team ensuring effective use of time and professional accountability.
  • To respond to general public and other services regarding enquiries to referral protocol, offering professional advice and education on service provision as required.
  • To build links with referrers and manage links with all referring agencies.
  • To provide advice and support to carers.
  • To ensure the team safeguards vulnerable individuals.
  • To monitor and evaluate the quality of care given to service users.
  • To organise and chair multi-agency and multi-professional meetings with complex case management when appropriate.
  • Ensure that the principle of continuity of care is maintained and that appropriate plans are made for patients discharge and transfer from clinical area in line with Mental Health Act, CTP, the NHS and Community Care Act 1990 and The Mental Health Measure.
  • To work alongside the CMHT’s and inpatient wards to provide a seamless service, which meets the needs of people and their carers.
