Community Mental Health Nurse

Hays Healthcare are actively recruiting a Community Mental Health Practitioner or Community Mental Health Nurse to work in Havering, East London.

Rate: £23 - £28 per hour
Shifts: Days, 9:00 to 17:00 Monday - Friday

Your New Role
Join our team as a Community Mental Health Nurse and make a real difference in the lives of individuals in need. As a compassionate and skilled nurse, you will provide high-quality mental health care and support to patients within the community. This role offers a unique opportunity to work collaboratively with patients, their families, and multidisciplinary teams to promote recovery and enhance overall well-being.
Key Responsibilities
Assess and evaluate the mental health needs of patients, developing and implementing individualised care plans.
Deliver evidence-based interventions and treatments, providing ongoing monitoring and support.
Collaborate with community organisations and resources to facilitate access to additional services and support.
Educate and empower patients and their families on mental health, self-care, and coping strategies.
Act as an advocate for patients, ensuring their rights and preferences are respected in all aspects of care.
Contribute to the development and implementation of mental health initiatives and programs within the community.
