CHC Nurse Assessor

Search are currently looking for Band 6/7 Nurse Assessor to work within the Continuing Healthcare team.

Ideally they would like someone full time. Monday-Friday, 09:00-17:00. They may accept less days/hours alterations if you can commit to a minimum of 4 shifts each week.

Duties:

* Compiling and completion of robust clinical assessments of patients with complex health needs such as fast track end of life assessments, CHC reviews, Checklists & DST's.
* Identifying patients at risk and ensuring that appropriate safeguarding procedures are implemented.
* Attend & contribute with MDT meetings, and best interest meetings, ensuring excellent verbal communication is maintained & clear, concise documentation is completed within a short time frame capturing the key elements of the meetings.

* Attend weekly team meetings.
* Adapting to new structural changes & methods of working due to internal changes within the CSU.
* Ensuring that my statutory and mandatory training is updated.
* Working remotely with nursing home managers when care homes would close down or restructure to a residential care home from a nursing care home, ensuring each patient had a timely & robust assessment completing to identify their individual care needs prior to moving home.
