Job Title: Care Coordinator

Salary: Up to £30,000 PA DOE

Location: South London

This is an excellent opportunity for an experienced care coordinator to join a leading independent home care provider within their South London branch and support the delivery of personalised, high-quality self-funded care.

Our client deliver up to 2000 hours of care per week, and have recently been inspected as Outstanding by CQC.

Requirements

  • Ideally NVQ Level 2 or 3
  • Previous experience within a scheduling role
  • Knowledge of CQC compliance is essential
  • Good working knowledge of IT Systems
  • Ideally experienced in conducting care assessments, care reviews and being able to motivate care staff communicate effectively and efficiently via phone, email and in-person meeting

Responsibilities

  • Day to day management of schedules around caregivers and clients.
  • Working with clients and their families
  • Ensure care delivered is compliant with all relevant legislation
  • Undertake assessments and reviews
  • Undertake supervisions and direct observations
  • Improving and maintaining the continuity of care

Remuneration and Benefits:

  • Up to £30,000 PA DOE
