Job Title: Care Coordinator
Location: Neath
Salary: Up to £30,000 per annum
Working Hours: Monday - Friday, 8:00 AM to 4:00 PM


We are currently seeking a dedicated and experienced individual to join a fast growing domiciliary care organisation as a Care Coordinator in Neath. As a Care Coordinator, you will play a crucial role in ensuring the effective coordination of care services to enhance the quality of life for the clients.


Key Responsibilities:

  • Coordinate and schedule care services for clients in a timely and efficient manner.
  • Liaise with care staff and clients to ensure service delivery meets the highest standards.
  • Conduct assessments and reviews to determine the appropriate level of care required.
  • Maintain accurate records of client information and care plans.
  • Collaborate with healthcare professionals to provide comprehensive care solutions.
  • Respond to queries and concerns from clients and their families.

Qualifications and Experience:

  • Previous experience in care coordinating is essential.
  • Full UK driver's license is mandatory.
  • Strong organisational and communication skills.
  • Ability to work independently and collaboratively in a team.
  • Knowledge of care regulations and best practices.

Working Hours:
This is a full-time position with working hours from Monday to Friday, 8:00 AM to 4:00 PM.


Salary:
The salary for this position is up to £30,000 per annum, depending on experience and qualifications.
