This GP Surgery is a 2 partner training practice, providing excellent quality health care and is a consistently high QoF achiever.

They are expanding their nursing team and are looking for a full-time/part-time Advanced Nurse Practitioner to join their existing professional and friendly team of 6 Advanced Nurse Practitioners, 6 Practice Nurses and 2 HCAs.

The position includes telephone triage, working with the duty team and holding own clinics.

Applicants will need to have the relevant Master/degree qualifications or equivalent, be an independent nurse prescriber and be on the NMC register.

Previous experience of working in General Practice and knowledge of SystmOne are desirable, however they have a local roadmap in place for transition from secondary to primary care.

Main duties of the job:

  • The position includes working as a member of the duty team, completing telephone triage and holding own clinics.
  • Provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
  • Demonstrate safe, clinical decision-making and expert care for patients within the general practice and their own homes.
  • Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

The successful applicant must be able to:

  • Work autonomously
  • Provide care that is based on best practice and up to date clinical evidence
  • Take a comprehensive history
  • Carry out in depth assessments
  • Use expert knowledge and clinical judgement to identify the potential diagnosis
  • Refer patients for investigations where appropriate and make a final diagnosis
  • Decide on and carry out treatment, including the prescribing of medicines or refer patients to the appropriate specialist
  • Assess and evaluate with patients the effectiveness of the treatment and care provided and make changes as needed
  • Chronic disease management.

Person Specification - Experience

Essential

  • Minimum of 2 years post registration experience
  • Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the ANP role
  • Experience in management long term conditions e.g. asthma, COPD, diabetes, CHD
  • Evidence of working autonomously and as part of a team
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice

Desirable

  • Interpreting and implementing local and National policy agendas for health

Qualifications

Essential

  • Registered General Nurse (Currently registered with the Nursing & Midwifery Council)
  • Recognised ANP qualification at minimum of degree level or equivalent or Masters

Desirable

  • Independent Nurse Prescriber
  • Teaching / Mentoring experience and /or qualification
