Bristol City Council are seeking Social Workers to join their Discharge to Assess Team.

Whilst the team are mostly home based, there may be a need for face to face assessments within the community - candidates must be aware of this.
full time

The individual will need to be exceptionally strong in their understanding of hospital discharge and safeguarding.

Experience in "delay transfer to care" is ideal but not essential.

Saturday working is available for those who would like a day off during the week.
