Adult Independant Living Worker

Seven Social Care is looking for a Adult Independent Living Worker to fill an exclusive opportunity in Hammersmith & Fulham.

We have opportunities available for up to 12 months and the pay is £24.96 (Umbrella) per hour.

The Hiring manager is looking for someone who can work 36 hours a week (various shifts available from Monday to Sunday).

The role:

The role holder must ensure that residents are at the heart of the strength-based conversations and planning process, finding creative solutions that enables adults to regain or maintain independence, access outcome focused support, and experience a seamless journey through the care system.

They work in partnership with residents and carers involving professional staff and other agencies and service providers to establish arrangements and services that meet their assessed needs and promote independence, choice, and control. Fundamental to achieving this, the role requires an understanding and application of strength based, asset-based approaches in service delivery and practice.  

Key Responsibilities:

The Community Response and Reablement Team which provides information, advice, community social work, occupational therapy and reablement services for new people in the community.

The Community Independence and Wellbeing Team which provides community social work and occupational therapy for existing people living in a range of care and support settings.

The Hospital Team providing 7 days 8:00am - 8:00pm social work and social care support to individuals, carers and families involved in discharge from hospital, or in

Community Team for people with learning disabilities

Community and hospital teams for people with mental health needs
