We have a great opportunity in our Lucy Adams team for a Support Worker (Part-time - 25.9 hours per week) to deliver Housing Related Support to residents within Lucy Adams services by ensuring completion of person-centred risk assessments and support plans. You will be required to refer to and work alongside partner support agencies to effectively cater for all support needs presented by residents, with the aim to support and enable residents to maximise their potential and move on successfully to independent living accommodation.

Part-time position of 25.9 hours per week - we can be flexible on working patterns.

Requirements

Outline of Key Responsibilities...

  • Provision of a person-centred approach when delivering and reviewing Support Plans and Risk Assessments for each resident within the organisations policies and procedures
  • To promote, support and ensure fair access and inclusion for each resident at every point throughout the service and to ensure each resident has the opportunity to maximise their potential and achieve positive outcomes
  • To contribute to team process of new referrals and completing risk assessments with potential applicants for discussion as a team
  • Encourage, accompany and assist residents to access services where appropriate and identified in their Support Plan
  • Sourcing and maintaining effective networks with statutory and other external agencies to benefit residents

We are looking for someone who has...

  • Experience in relating effectively with individuals in an empathetic way
  • Experience of working with individuals with mental health, drug or alcohol related issues
  • Excellent communication and interpersonal skills
  • Awareness of importance of confidentiality and data protection
  • Knowledge of issues leading to homelessness
  • Computer literacy and ability to learn new IT systems
  • Current Driving Licence and access to own vehicle
