Overview:
As a Support Practitioner, you will join our client's dynamic team, playing a crucial role in supporting individuals to achieve their personal goals and aspirations. By following each person's unique support plan, you will provide practical assistance across various aspects of daily life, ensuring their well-being and fulfilment.

Key Responsibilities:

  • Create, update, and review support plans based on evidence and individual needs.
  • Use problem-solving skills to suggest changes that increase choices and enhance social networks.
  • Build effective relationships with families, friends, and professionals to advocate for individuals rights and needs.
  • Follow human-rights-based processes to ensure access to information and complete necessary documentation.
  • Manage time and workload effectively to meet individuals needs while maintaining high standards of care.

Training & Qualifications:
Our client requires Support Practitioners to maintain appropriate professional registrations and achieve relevant qualifications. To support you with this they fully fund your SVQ with all training completed within paid work time.
