Job Opportunity: Social Worker (Adult Localities Team)

Position: Social Worker (Adult Localities Team)
Location: Doncaster Council
Salary: £41,438 per annum
Contract: Permanent, Full-time

Responsibilities:

• Undertake holistic assessments of adults referred to the Adult Localities Team, identifying their strengths, needs, and desired outcomes.
• Develop and implement care and support plans tailored to meet the individual needs and goals of service users, in collaboration with the multi-disciplinary team and external partners.
• Provide ongoing support, guidance, and advocacy to adults, empowering them to make informed decisions and achieve greater independence and well-being.
• Work closely with health professionals, care providers, housing agencies, and other stakeholders to coordinate services and ensure seamless transitions between different care settings.
• Safeguard vulnerable adults at risk of abuse, neglect, or exploitation, taking appropriate safeguarding measures and liaising with safeguarding teams and relevant authorities as required.
• Maintain accurate and up-to-date case records, reports, and documentation in accordance with statutory requirements and departmental policies.
• Participate in regular supervision, team meetings, and professional development activities to enhance practice skills and knowledge.
• Engage in reflective practice, seeking guidance and support from senior colleagues to address complex cases and challenging situations effectively.

Requirements:

• Qualified Social Worker registered with Social Work England.
• Proven experience working within adult social care services, with a strong understanding of relevant legislation, policies, and procedures.
• Excellent assessment, communication, and interpersonal skills, with the ability to build positive relationships and engage effectively with service users and their families.
• Ability to work autonomously and collaboratively as part of a multi-disciplinary team, prioritizing workload and managing competing demands in a dynamic environment.
• Commitment to promoting the rights, dignity, and well-being of adults, respecting their autonomy and facilitating their participation in decision-making processes.
• Knowledge of person-centered approaches and strength-based practice principles, with a focus on empowering individuals to lead fulfilling lives.
• Valid driving license and access to own vehicle for work-related travel (where applicable).
