Senior Support Worker/Team Leader

Start date - ASAP

Location - Hammersmith

Contract - Initially 3 months

Hours per week - 36 hours per week

Rate of pay - £27.96ltd per hour

About the Role

The London Borough of Hammersmith and Fulham are seeking a highly motivated and experienced Team Leader to join their team. The successful candidate will be responsible for providing excellent support to vulnerable adults with a range of disabilities, including physical disability, autism, challenging behaviour, mental health, and dementia.

Main Responsibilities
  • Deliver excellent support to vulnerable adults with a range of disabilities
  • Work in partnership with schools, colleges, and other service providers to deliver transition groups for residents
  • Assist residents in identifying progression opportunities and provide ongoing transition support
  • Establish positive relationships with residents and support them to increase their confidence and realise their potential
  • Explore services such as independent living establishments and a curriculum for independent living skills
Requirements
  • Proven experience within a supervisor position or in a similar role
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organisational and time-management skills
  • Experience working with vulnerable adults with disabilities
