Salary: £37,336-£44,428 (dependent on experience)
Working Pattern: Permanent, full time
Location: Horsham or Crawley

We are recruiting for a qualified Social Worker to join our Primary Care Network - Multi Disciplinary team.

In the North of West Sussex we work collaboratively with our NHS and third sector colleagues to prevent and reduce people from requiring hospital admissions and to live their best life in the community. There are 6 Primary Care Network Multidisciplinary Teams in the main towns of Burgess Hill, Haywards Heath, East Grinstead, Crawley, and Horsham.

Working as a Social Worker in a busy community team, your role will involve working with adults from 18 years and upwards with physical disabilities, acquired brain injuries, chronic health conditions, older people and people with dementia. Tasks will include assessments, reviews, safeguarding enquiries and mental capacity act assessments.
