Job Description:

Are you an experienced social care professional with a passion for supporting young adults with disabilities during their transition to adulthood? Harrow Council is seeking a dedicated Senior Practitioner to join our CYADS (Children & Young Adults Disabilities Services) 18-25 Transition Team. As a Senior Practitioner, you will play a pivotal role in providing guidance, support, and management oversight within our dynamic team.

Key Responsibilities:

  • Provide expert advice and support to social workers and care managers on casework, ensuring compliance with the Care Act 2014 and Children's Act 1989.
  • Manage duty responsibilities, including chairing Professional Meetings/Case Conferences and ensuring timely response to incoming concerns.
  • Conduct complex enquiries, collaborating with internal and external stakeholders as necessary.
  • Supervise and mentor up to four team members, fostering a culture of excellence and ethical practice.
  • Collaborate with other internal services such as Enablement, Shared Lives, and SEN to deliver holistic support to service users.
  • Participate in milestone-based approaches for young adults with EHCPs, including attending important annual reviews and school/college events.
  • Assess training needs within the team and facilitate relevant training sessions to enhance service delivery.
  • Maintain accurate records on the Mosaic database and ensure thorough, cost-effective decision-making in all casework.

Qualifications and Skills:

  • Demonstrated experience in social care practice, with a strong understanding of relevant legislation and policies.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams.
  • Proven leadership abilities, including the supervision and development of staff.
  • Sound judgment and decision-making skills, with a commitment to person-centered approaches.
  • Willingness to work occasional unsocial hours and conduct visits in the community as required.
