Contract Type: Locum, 3-Months

Main Duties:

  • Establishing effective communication and referral pathways those individuals who are homeless and in contact with other health and care agencies
  • Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working, attending multi-disciplinary meetings when as and when required
  • Supporting service users from point of entry into the service and through their treatment/recovery journey
  • Providing screening, assessment, and recovery planning and onward referral
  • Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided. Promoting carer, service user and community involvement
  • Providing advocacy for access to partnership services
  • Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination

Requirements for the role of Rough Sleepers Recovery Coordinator:

  • Recent experience in Substance Misuse, Substance Abuse, Addiction, Drug and Alcohol, Needle Exchange, or Recovery.
  • Relevant qualifications or certifications.
